Church check request form

Eklo’s Check Request template is a church check request form for invoices, honorariums and reimbursements. It asks who to pay, how much, what it’s for and how the check should be delivered, and requires the invoice or receipt. Every request gets two approvals: one to confirm the expense, then finance to release the check.

The approval path

Steps run in order, and a decline ends the request, so later steps don’t run. When you install the template, you choose who decides each one. Nobody can approve their own request: when nobody else can decide a step, your workspace owners and admins decide it.

  1. Approval

First approver. Confirms the expense is expected and budgeted.

By default, anyone with the Admin role decides.

    Every request.

  2. Finance release

    Finance approver. Releases the check.

By default, anyone with the Owner role decides.

Every request that gets this far.

How approvals work

Who it suits

  • Churches that still cut paper checks for vendors, guest speakers and reimbursements.
  • The bookkeeper who needs documentation and a budget line before writing a check.
  • Staff who want to know their check is on its way without walking to the finance office.

Questions

Does Eklo print or send checks?

No. Eklo collects the request, the invoice and the approvals. Finance writes the check the way it does now, and the request records who released it and when.

Why does every check need two approvals?

Two people seeing every payment is a basic financial control. The first approver confirms the expense was expected; finance confirms there’s documentation and releases the check. You can change either step.

Can someone submit a check request without an invoice?

No. The invoice or receipt is a required question, with up to five files. The form also asks whether this is a new vendor who needs a W-9.

Does Eklo connect to our accounting software?

No. The form’s editors can click Export CSV on its page to download every check request as a spreadsheet for your bookkeeper.
