Request forms and approvals for church staff.
Eklo Forms is where a church takes requests: facility bookings, events, purchases, time off. Each request gets a reference number, runs through the approval path you set, and can become a task and a chat post once it’s approved.
Replaces Jotform
Jotform keeps a form and its approvals in Jotform. Eklo keeps the request, who approves it, the task it becomes and the conversation about it in the workspace your staff already use. Jotform’s Bronze, Silver and Gold plans list at $39, $49 and $129 a month (October 2026), and Jotform gives churches 50% off. Eklo is one flat price per church, with Tasks and Chat included.
Build a form
Start from a blank form or a template, and ask only what you need.
Thirteen kinds of question
Short and long text, number, money, date and time, yes or no, dropdown, multi-select, person, file upload, email, phone and web address.
Conditional questions
Show a question only for some answers, like a setup question that appears only when the room is the sanctuary.
Steps and sections
Split a long form into pages, and group questions under headings.
Preview before you publish
Try the form as people will see it. Nothing you submit in the preview is saved.
Who can see it
Make a form public, open to the whole workspace, for one team, or for the people you pick.
Close it when you’re done
Close a form straight away, or set a date and time for it to close.
Nine church templates
Each template comes with its questions and approval steps. Pick who approves each step, install it, and change anything you like.
Approval paths
A form’s workflow runs every time someone submits it. Add the steps in the order you want.
Who approves each step
Anyone in the workspace, everyone with a role or a title, one person, a team’s admins or anyone on a team.
Rules for each step
Run a step only when the answers match, like a second sign-off when the amount is over $500.
Then the work starts
A workflow can also create a task in a project, post to a channel, and send an email or a notification.
No approving your own request
Nobody can approve their own submission. If they’re the only one who could, the workspace’s owners and admins decide instead.
Public links
Some requests come from people who aren’t on staff. A public form takes them without an Eklo account.
A link and a QR code
Copy the link for your website, bulletin or email, or download a QR code for a slide or a printed sign.
No account needed
People fill it in, pass a quick “I’m human” check, and get a reference number and a copy of their answers.
They hear back
The approval result and any questions go to the email address they give.
Kept off the public link
File uploads, person questions and custom fields aren’t shown to people outside your workspace.
Ask before you decide
When an approver needs more before deciding, they ask the submitter in Eklo instead of starting an email thread.
Request info
Type a question and, if you like, let the submitter change some of their answers.
Answered from a link
The submitter gets an email with a link to answer. Someone who used a public form answers without signing in.
Every change shown
The answer comes back to the approver with what changed, like the event date moving from April 5 to April 12.
Status tracking
Everyone can see where a request stands without asking.
Reference numbers
Each request gets one, like FAC-0001, with the letters you choose for each form.
Waiting on me
Approvers see what’s waiting on them, how long it has waited and the step it’s at.
My submissions
Staff see each of their requests: waiting on someone, waiting on their answer, approved or declined.
A timeline for every request
Each submission shows what has happened so far. A form’s editors can export its submissions as a spreadsheet.
Questions
What is Eklo Forms?
Eklo Forms is where a church takes requests: facility bookings, events, purchases, time off. Each request gets a reference number, runs through the approval path you set, and can become a task and a chat post once it’s approved.
Can people outside our staff submit a request?
Yes. Make the form public and share its link or QR code. People fill it in without an Eklo account, and the approval result goes to the email address they give.
How do approval paths work?
Each form has a workflow that runs when someone submits it. Add approval steps, choose who decides each one, and add rules so a step runs only for some answers. When a request is approved, the workflow carries on: it can create a task, post to a channel and send messages.
Which templates come with Eklo?
Facility Request, Event Request, Purchase Request, Check Request, Creative and Communications Request, Time Off Request, Volunteer Application, Background Check Authorization and Incident Report. Each comes with its questions and approval steps, ready to change.
Can an approver ask a question before deciding?
Yes. Request info emails the submitter a question. They answer from the link in the email, and can update any answers the approver offered.
Does Eklo replace Jotform?
For church requests, yes. Eklo keeps the form, its approvals, the task it becomes and the conversation about it in one workspace, at one flat price per church that also covers Tasks and Chat.
