Church communications request form
Eklo’s Creative and Communications Request template is a church communications request form. Ministries use it to ask for graphics, video, print, social posts, web updates and stage announcements. It asks what’s needed, what it must say, when it’s due and who approves the content, and sends it to your communications lead.
- 12 questions

The approval path
Steps run in order, and a decline ends the request, so later steps don’t run. When you install the template, you choose who decides each one. Nobody can approve their own request: when nobody else can decide a step, your workspace owners and admins decide it.
Communications intake
Communications lead. Accepts the request and schedules the work.
By default, anyone with the Admin role decides.
Every request.
Who it suits
- Communications teams who get requests in hallway conversations and a week too late.
- Churches with one designer or a volunteer creative team serving every ministry.
- Ministry leaders who want to know what to send and how much lead time to give.

Questions
How much lead time should people give?
The form tells them: two weeks is typical for graphics, four for video and print. Change that wording in the builder to match your team.
Does approving the request approve the design?
No. The approval step is intake: your communications lead accepts the request and schedules the work. The form asks who gives final approval on the content, so your team knows who to send drafts to.
Can the communications team track the work as tasks?
Yes. Add a workflow step after intake that creates a task in your communications project, with the request embedded in it.
Can people attach files?
Yes, up to five reference files or existing assets, plus a reference link. File questions need people to be signed in, so this form is for staff, not a public link.
