Church facility request form
Eklo’s Facility Request template is a facility use request form for church staff and ministry leaders. It asks what the space is for, when, which rooms and what setup or A/V is needed, then sends every request to facilities for approval.

The approval path
Steps run in order, and a decline ends the request, so later steps don’t run. When you install the template, you choose who decides each one. Nobody can approve their own request: when nobody else can decide a step, your workspace owners and admins decide it.

Facilities approver. Confirms the space is free and setup can be covered.
By default, anyone with the Admin role decides.

Who approves a facility request?
Anyone with the Admin role, or your owners if nobody else has it yet, until you choose someone else. When you install the template you can pick one person, everyone with a title like “Facilities Manager”, a team’s admins or the submitter’s manager.
Does Eklo check the room for conflicts?
No. Eklo doesn’t keep a room calendar. The facilities approver checks the space is free before approving, and the request keeps the dates, the rooms and the decision together.
How do recurring bookings work?
The form asks whether the booking is recurring, and if it is, how often and until when. One request covers the whole series, so facilities approves it once.
Can I change the rooms on the list?
Yes. The template installs as a form you own. Open it in the builder and rename, add or remove rooms to match your building.
